Sutureless Scleral Fixation of Intraocular Lens Implantation Using Yamane’s Technique in Timor-Leste: Initial Experience

I am a young Ophthalmologist recently graduated from South Korea. Currently working at Eye department of National Hospital Guido Valadares, Timor-Leste. My current job is providing eye health care to the people and providing surgery such as small incision cataract surgery (SICS), pterygium surgery, and other minor surgery like cyst and chalazion removal.

I am also actively participating in eye camp outreach in Timor-Leste to provide eye consultation and surgery in remote areas, especially to those who cannot get access to the eye health care in the hospital.

I am passionate about Retina.

Abstract:

Purpose

To report first successful case of intraocular lens scleral fixation (SFIOL) implantation using Yamane’s technique performed at eye department of Hospital Nacional Guido Valadares, Timor-Leste.

Methods

Interventional case report in one patient with dislocated IOL following ocular trauma.

Results

We performed SFIOL using Yamane’s technique in a 55-year-old woman, with dislocated IOL in OD. The visual acuity (VA) was FC 1/2 m in OD and 6/9 in OS. Her medical history included cataract surgery performed 4 years earlier. 5 mm scleral tunnel superiorly was created to removed the previous rigid polymethylmethacrylate (PMMA) three-piece IOL and anterior vitrectomy was performed. A bimanual approach with a 27-gauge needles were inserted on the 8 and 2 o’clock position, 2 mm posterior from the limbus, and kelman forcep was used to threaded the haptics into the needles. Both haptics then cauterized and were externalized onto the conjunctiva. The best-corrected visual acuity (BCVA) on the first day was 6/19, and the intraocular pressure was 11. The IOL is centered, the haptics well covered. The BCVA on the first week and one month follow-up were 6/12 and 6/9 respectively. No post operation complications were found.

Conclusion

Yamane scleral fixation IOL implantation technique can be considered as treatment of choice on patients without adequate capsular support as a viable, and perhaps preferable option for patients requiring secondary IOL placement in Timor-Leste.
