Achieving Global Consistency and Equitable Access: Early Experiences from the First Year of the Ophthalmology Foundation Exams

Abstract:
PURPOSE The Ophthalmology Foundation (OF) exams were launched to provide an internationally recognised, standardised written assessment of ophthalmic knowledge and clinical reasoning. This initiative aims to enhance global consistency in training, standardise ophthalmic assessment worldwide, and promote equitable access to high-quality certification, ultimately contributing to improved eye care standards globally.
METHODS To support access, examination fees are adjusted according to the World Bank country income index, and online delivery allows trainees to sit the exams within their own institutions, reducing travel and associated costs. Results are issued within 14 days, enabling candidates to progress efficiently through the OF or their local certification processes.
RESULTS 2025 is the first year with two sittings in 42 centres in multiple regions. Feedback from candidates, hosts, examiners, educationalists, standard setters, and question writers to ensures a robust, fair, and easily accessible examination process. Participants reported high satisfaction with the exam content, structure, and global relevance. Examiner feedback highlighted strengthened calibration and consistency. Early challenges included variations in digital infrastructure, time zone coordination, and the need for wider awareness and preparatory resources.
CONCLUSION The first year was highly successful with significant progress in establishing a globally standardised, equitable, and accessible ophthalmic examination. Work continues on identifying more regional hosts, expanding online delivery, and enhancing regional collaboration will further strengthen equity, sustainability, and global eye care standards.
